Please be advised that our Integrative Health Transitions of Care / ER Follow-up Clinic location in The Woodlands will permanently close effective 1/23/2026.

After this date, medical services will no longer be available at this address.

• Patients are encouraged to seek care with another transitional or urgent care clinic.
• Each patient’s Insurance company should be able to assist with information on in-network locations for their care.
• Patients of CommonSpirit / Saint Luke’s also have the option to utilize their Find a Doctor links on their website.
